Buying medicine on Koh Phangan is easy and cheap, though thinner and more spread out than on Koh Samui or Phuket. Thong Sala, the island's pier town, holds the densest cluster of options - independent chemists, the island's only Watsons branch, and First Western Hospital's private pharmacy - while Srithanu, Chaloklum and Ban Tai each have at least one local option, and Haad Rin leans on 7-Eleven basics and island-wide delivery. Thai pharmacists can sell many medicines directly that would need a prescription back home; the main caveats here are no airport, no rail or mass transit, and thinner stock of specialist drugs than the bigger islands. Here is how the system works, where to go, what you can and cannot buy over the counter, and what common medicines cost.

Where to buy medicine in Koh Phangan

Independent Thai pharmaciesEveryday

Small independent chemists handle most everyday medicine on Koh Phangan, concentrated in Thong Sala - Center Pharmacy on Taladmai Road, The Chemist Pharmacy nearby at Moo 1, and Thong Sala Pharmacy in the town centre. They are cheap, quick, and Thai pharmacists have wide latitude to sell many medicines directly that would need a prescription back home. Look for the green cross; the busier Thong Sala shops tend to have a pharmacist on duty and reasonable English.

Watsons (no Boots on the island)Chain

Watsons has a branch on Koh Phangan for genuine branded OTC medicines, vitamins, sun care and personal care, open roughly 8:30am-9pm daily. Unlike Koh Samui or Phuket, Koh Phangan does not currently have a Boots store - Watsons is the only international health-and-beauty chain on the island, so it is worth checking there first for anything branded before assuming you will need to ferry to Koh Samui.

Hospital pharmaciesPrescriptions

Koh Phangan Hospital (the government hospital, near Wok Tum/Srithanu just north of Thong Sala) runs a low-cost public pharmacy. First Western Hospital in Thong Sala - a private, fully licensed hospital with an on-site lab and CT scanner - dispenses from its own pharmacy and is the most capable option for anything needing a proper prescription. Bandon International Hospital's small Koh Phangan branch clinic in Baan Tai also dispenses basic medication with 24-hour emergency call-out. Use hospital pharmacies for controlled or specialist medication and anything prescribed on-site.

Village & area pharmaciesAround the island

Away from Thong Sala, Nopporn Pharmacy serves the Chaloklum fishing village and north-coast beaches with medical care and first-aid supplies, and Center Pharmacy 2 in Srithanu covers the yoga and wellness community at the main crossroads near the 7-Eleven towards Haad Yao and Mae Haad. Haad Rin itself has no well-known standalone pharmacy - residents there lean on 7-Eleven basics, island delivery, or a short ride to Thong Sala.

Island-wide deliveryConvenient

Because Koh Phangan has no rail or mass transit and areas are spread across a mountainous island, delivery services such as Vayana run pharmacy runs and prescription handling from Thong Sala out to Haad Rin, Srithanu and Chaloklum, with 24/7 availability advertised. Useful if you are unwell, based away from Thong Sala, or riding out the reversed monsoon season - though a first purchase of anything unfamiliar is still worth a face-to-face chat with a pharmacist.

Pharmacies by area

Thong SalaBest choice & most options

The island's pier town and administrative centre has the densest cluster of pharmacies - Center Pharmacy, The Chemist Pharmacy and Thong Sala Pharmacy - plus the only Watsons branch and First Western Hospital's private pharmacy. It is also the most central point to reach Koh Phangan Hospital, just to the north between Wok Tum and Srithanu. If you only remember one place to go, this is it.

Haad RinThinnest on-island options

The Full Moon Party beach on the island's southeast tip has 7-Eleven stores for basic first-aid items but no well-documented standalone pharmacy. Stock up in Thong Sala before heading down for a big weekend, rely on island delivery, or budget 20-30 minutes back to Thong Sala for anything beyond the basics.

SrithanuYoga & wellness belt

The west-coast wellness hub is served by Center Pharmacy 2 at the main crossroads towards Haad Yao and Mae Haad, alongside a 7-Eleven and bakery at the same junction - convenient for the retreat and long-stay yoga community based on this side of the island.

Chaloklum & the north coastFishing-village north shore

Nopporn Pharmacy covers Chaloklum's working fishing village and the quieter north-coast beaches nearby (including Bottle Beach access points), combining medicine with basic first-aid and medical care for a part of the island well away from Thong Sala.

Ban Tai / Ban KaiEast-coast backup

Bandon International Hospital's small 9-bed branch clinic near the Ban Tai 7-Eleven (opposite Luna Cafe) gives the east-coast strip between Thong Sala and Haad Rin a 24-hour emergency call-out and basic dispensing option without the full drive to town.

Prescriptions, safety & practical tips

Over the counter vs prescriptionKey rule

Thailand is far more relaxed than Western countries: many medicines that need a prescription at home - including a lot of antibiotics and everyday drugs - can be bought directly from a Koh Phangan pharmacist after a quick chat. Genuinely controlled medicines (strong painkillers, sedatives, psychiatric and some sleep drugs, and ADHD stimulants, treated as narcotics in Thailand) still require a doctor's prescription, best obtained through First Western Hospital, Koh Phangan Hospital or the Bandon International branch clinic.

Bring your generic nameSmart tip

Brand names differ between countries, so note the generic (chemical) name and dose of anything you take regularly - for example metformin 500mg rather than a home brand name. Pharmacists recognise generics instantly, which matters on a smaller island where a specific imported brand may not be stocked. A photo of your current packaging or a doctor's note helps, especially for ongoing prescriptions.

Genuine products & storageSafety

Stick to Watsons, hospital pharmacies and the established Thong Sala chemists to avoid counterfeit or poorly stored stock - avoid market stalls or unlabelled sources. Check expiry dates, and remember the island's heat and humidity affect storage: buy heat-sensitive items such as some antibiotics and insulin from a pharmacy or hospital with proper refrigeration rather than a small village shop.

No airport & island stockPlan ahead

Koh Phangan has no airport and no rail or mass transit, so a less common or specialist drug may need ordering, a ferry to Koh Samui (roughly 40-60 minutes) or the mainland at Surat Thani, or onward travel to Bangkok. If you rely on a specific brand or specialist medication, bring a buffer supply, confirm availability with a Thong Sala pharmacy or hospital early, and set up a repeat prescription through First Western Hospital or Koh Phangan Hospital rather than assuming a village pharmacy will stock it.

Travel & customsArriving

You can bring a personal supply of prescription medicine into Thailand - carry it in original packaging with a doctor's letter, and keep quantities reasonable (a common guide is up to about 30 days for controlled drugs, with documentation). Some psychotropic and narcotic medicines are restricted or banned, so check before you travel; once here, First Western Hospital or Koh Phangan Hospital can usually arrange a local equivalent.

FAQ

Koh Phangan pharmacy FAQ

Do you need a prescription to buy medicine in Koh Phangan?

Often not. Thai pharmacies have wide latitude and will sell many medicines - including a lot of antibiotics and everyday drugs that need a prescription at home - directly after a short conversation with the pharmacist, at Watsons or an independent chemist like Center Pharmacy or The Chemist Pharmacy in Thong Sala. Genuinely controlled medicines such as strong painkillers, sedatives, some psychiatric and sleep drugs, and ADHD stimulants (treated as narcotics in Thailand) still require a doctor's prescription, best obtained at First Western Hospital, Koh Phangan Hospital or Bandon International Hospital's branch clinic.

Is there 24-hour pharmacy access on Koh Phangan?

Effectively yes for anything urgent. First Western Hospital and Koh Phangan Hospital both run 24-hour emergency departments that can dispense medication outside normal hours, and Bandon International Hospital's Baan Tai branch offers 24-hour emergency call-out. Standalone chemists like Center Pharmacy keep normal daytime-to-evening hours, so for the middle of the night, a hospital pharmacy or an island delivery service such as Vayana is the more reliable route.

Is there a Boots or Watsons on Koh Phangan?

There is a Watsons branch on the island, open roughly 8:30am to 9pm daily, for genuine branded OTC medicines, vitamins, sun care and personal care. Koh Phangan does not currently have a Boots store, unlike Koh Samui or Phuket - if you specifically need a Boots-branded product, that means a ferry to Koh Samui.

How much do common medicines cost in Koh Phangan?

Everyday medicines are cheap. A pack of paracetamol is roughly 20-50 THB, painkillers 40-90 THB, antihistamines 50-120 THB, a common antibiotic course around 150-400 THB and rehydration salts a few baht per sachet. Branded sunscreen and DEET repellent cost more (300-700 THB and 80-200 THB). A quick pharmacist consultation is usually free. As on any smaller island, prices can sit a touch above the mainland because stock is shipped in via ferry.

What if I need specialist or regular prescription medication not available on the island?

For common maintenance drugs, First Western Hospital or Koh Phangan Hospital can typically re-issue your prescription locally so refills are straightforward. Because Koh Phangan is a smaller island with no airport, a less common or specialist medicine may need to be ordered in, sourced on Koh Samui (a roughly 40-60 minute ferry away) or the mainland at Surat Thani, or brought from Bangkok. Bring a buffer supply for your first months, carry a doctor's letter and the generic name, and confirm availability early with a Thong Sala pharmacy or hospital.
